ADULT SOCIAL CARE MENTAL HEALTH HOUSING SUPPORT SERVICE:

Supports people with mental health problems to enable them to live in the community. Includes help in obtaining suitable accommodation, in maintaining tenancies and the provision of emotional support. Works mainly with people who wish to try living independently in a council or housing association property. Team includes a person who works with people who have both mental health problems and learning disabilities. Also a homelessness team which includes a social worker, women’s worker, ethnic minorities worker,and a generic worker and rough sleeper worker. Please note that the service does not have its own emergency accommodation and is not able to provide help with domestic chores.

Referral Criteria: Homeless Team – People 16 and over living in the Leeds catchement area who are homeless or at risk of homelessness and have mental health issues and wish to live independently. General Team – People 18 and over living in the Leeds catchement area who have a housing need and have mental health issues and wish to live independently. Referrals accepted from all sources.

Exclusions: None, unless risk assessment should inform of particular areas of concern.

ADULT SOCIAL CARE TRANSITIONAL HOUSING UNIT: Cottingley Court (LS11) – 0113 336 7727, Bewerley Croft(LS10) – 0113 336 7732, 144 Spen Lane THU (LS7) -0113 336 7878.                                                                                                                      Provides 6-8 month Tier 1 resettlement service with the aim of supporting people to live as independently as possible.

Service Users are offered self contained flat under a shorthold tenancy agreement with Progress Care Housing Association. Leeds City Council Adult Social Care provide 24/7 staffing on site. Services work collaboratively with service users on person centered, recovery led support planning, linking with existing and potential support networks, utilising specialist and mainstream resources. Each THU has a short term Respite flat – accessed directly through the THU’s – for which charges may apply. Each THU also has a Crisis Flat on each site [accessible only by the Crisis Assessment Team/Single Point of Access service/Intensive Community Services – 0300 300 1485].

Referral Criteria: Adults over 18 with ongoing involvement of a mental health professional and a formally diagnosis. PRIMARY: severe/enduring mental health with housing related support needs. CONCURRENTLY: Housing related support issues including homelessness; substance misuse; forensic service involvement; specialist involvement (including autistic spectrum services).

Exclusions: People with no formal diagnosis or formal mental health support in place; very high levels of substance use or chaotic/challenging behaviour; significant, active risk management concerns; those who are currently assessed as requiring long term supported accommodation (as services are time limited); those who do not currently engage with Mental Health support.

Provide recovery focused support in a therapeutic environment, intensive rehabilitation and prevention support to people with severe and enduring mental health problems or personality disorders.

Referral Criteria: Adults over 18 with enduring mental health needs, on enhanced CPA and under the care of a consultant psychiatrist or forensic psychiatrist; or under the care coordinated by the Personality Disorder (PD)  Network, people with high, complex or variable needs. This will include those defined as having a personality disorder or are challenging; involve drug and/or alcohol issues; or those who self-injure.

Exclusions: People in acute phase of illness or who need the security of an in-patient environment or forensic services, a primary diagnosis of drug/alcohol use or whose severity of drug/alcohol, or level of forensic history mitigates against communal living, not under the care of a consultant psychiatrist unless supported by PD Network.

Referral Criteria: Adults over 18 with a primary mental health diagnosis. Must be on CPA.

Exclusions: Non-engagement with service, risk levels that would impact on service users and service provision. Assessed on a case by case basis.

COMMUNITY LINKS – OAKWOOD HALL: 01132359079 http://www.commlinks.co.uk/oakwood-hall                                                               Oakwood Hall is a twelve-bed dual registered nursing and residential care home for people whose mental health support needs cannot be satisfactorily met from other services.

Referral Criteria: Adults over 18 with severe and enduring mental health health needs. Complex needs not easily met by existing services, reluctant to engage with services or experience frequent conflict with service providers, or be a risk through vulnerability or self neglect. Difficulties or a history of difficulties which exclude them from other services.

Exclusions: Where the needs of the individual can be met by another service, evidence of dementia, person has learning difficulties, person is experiencing an acute phase of illness, high level of physcial care is required or person has behaviours that could endanger local community.

It also provides satellite support to 12 people who are living independently in the local community. People are supported in their own homes but can also come to the project and access support and be part of the overall Rose Villa community.

Referral Criteria: Adults over 18 with a primary mental health diagnosis.

Exclusions: Non-engagement with service, risk levels that would impact on service users and service provision. Assessed on a case by case basis.

Supports people with mental health problems in Leeds, working on housing, employment and other needs to help with recovery.

The service is led by Community Links in partnership with Touchstone, Leeds Irish Health and Homes, St Anne’s Community Services and Leeds Federated Housing Association.

The service works with people to design a support plan around individual needs. They use the Outcomes Star assessment tool which shows the person’s journey of change through the service, which is reviewed regularly. Towards the end of the journey there are opportunities to become part of the Positive Pathways befriending and peer support service.

Before starting with the service a person can express a preference for both support provider and designated worker, and people remain in control of their support. The specialist worker preferences available are: Black African Caribbean, South Asian, Drug + Alcohol, Female/Male, Parent & Child.

‘Dry hostel’ project run by St Georges Crypt providing supported accommodation for men who want support to maintain abstinence from addictive substances/behaviours. All new residents expected to attend a full time structured day recovery program. Our in house program is called Growing Rooms which is supported by Spacious Places (see http://www.spaciousplaces.org.uk/whatwedo.html). Chaplain to the homeless available to speak to our clients at any time.

Referral Criteria: Men only. Must be willing to fit in with the recovery focus of the project and to do at least 15 hours of voluntary activity (i.e. recovery work, voluntary work or training/education). Must already be detoxed. Will take people on methadone or buprenorphine but they must be committed to reducing off this at an acceptable rate with support. Referral form available at bottom of webpage.

Exclusions: Residents initially given only a ‘respite bed’ and must wait at least two weeks and until staff are satisfied they are suitable before being offered a full licence agreement. Alcohol and breath tests performed regularly and lapses result in warnings and may lead to eviction. Staffing levels only allow us to take low clients who pose low to medium risk to others. Cannot take those with high risk or schedule one offenders.

Run by St Georges Crypt; Leeds’ only mens ‘wet’ hostel project – residents are allowed to drink alcohol on the premises but boundaries to this are put in place with the aim of encouraging them to gradually cut-down as well as identifying and addressing their needs.

Residents who have reduced their alcohol levels to a level where they can healthily consider ending their drinking are fully supported by care staff and the Leeds Detoxification Unit.  Once a resident is no longer alcohol dependent we discuss with them their options for future accommodation including our sister project Faith Lodge. All meals provided. Chaplain to the homeless available to speak to our clients at any time.

Referral Criteria:Men only. Only for those not yet ready to stop drinking. Not for those who have already stopped drinking and want to stay abstinent (please refer them to Faith Lodge instead). Referral form available at bottom of webpage.

Exclusions:

Staffing levels only allow us to take low clients who pose low to medium risk to others. Cannot take those with more serious history of violence or sex offences.

On interview or referral by an outside agency a key-worker will be allocated and a full support plan of action with targeted results will be mutually decided upon.  This is monitored on a regular basis and amended accordingly until the client is fully capable of managing all their affairs. The support offered includes help with budgeting, accessing benefits, dealing with difficult neighbours and all-round general well-being.

Referral Criteria: We look after clients who have or have not been through the Crypt’s emergency housing. Single men in the Leeds area.

Exclusions: Dependant on assessed risk. Home visits will not be conducted if a client is intoxicated when staff arrive.

Emergency Accommodation for single male or female over 16. Food, advice and practical support regarding access to housing, health and personal wellbeing are also available. Chaplain to the homeless available to speak to clients.

Referral Criteria: Referrals must be made via Housing Options. Out of office hours people can approach us and Housing will be contacted. Single males or females over 16.

Exclusions: If unable to compete an assessment due to be under the influence of drugs or alcohol.

 

ST ANNE’S MENTAL HEALTH SERVICE – 0113 243 2537                                 http://www.st-annes.org.uk/in_your_area/West_Yorkshire/Leeds                                       The service offers 49 single tenancies within a range of shared supported accommodation. Our aim is to create a secure environment which respects personal privacy and promotes recovery, independence, abilities.

Referral Criteria: Adults with serious and enduring mental health problems who have a primary mental health diagnosis. Identified support needs have  been assessed as meeting the Fair Access to Care Services criteria by Leeds Adult Social Care. Applicants must be single and have no children who need to live with them. Applicants must not present a significant risk to staff, other clients or property. Referrals accepted from any source.

Exclusions: No automatic exclusions. Care and support needs will be assessed against the level of support which is available. This will include looking at patterns of abusive and aggressive behaviour and any incidents of arson.
